Painful sex after childbirth: what’s normal and what’s not
Some discomfort is common in the first months after birth. Here is when pain needs a check-up.
Some pain during sex is common for a few months after giving birth, especially after a tear, stitches or while breastfeeding, which can cause vaginal dryness. It usually improves with time, lubricant and gentle return to sex. Pain that lasts beyond three to six months, or is severe, should be checked by a doctor.
When is it safe to have sex after childbirth?
There is no fixed rule, but many doctors suggest waiting until bleeding has stopped and any tears or stitches have healed, usually around four to six weeks. Wait until you feel physically and emotionally ready; there is no need to rush. Remember that you can get pregnant before your first period returns, so plan contraception.
Why can sex hurt after having a baby?
- Healing tears, an episiotomy scar or stitches
- Vaginal dryness from low oestrogen while breastfeeding
- Tight or weak pelvic floor muscles
- Tiredness and anxiety, which make it harder to relax and become aroused
- Less commonly, infection or a scar that has not healed well
What can make sex more comfortable?
Use plenty of water- or silicone-based lubricant, take time for foreplay, and try positions where you control the depth and pace. Pelvic floor exercises and physiotherapy help many women. If dryness persists after breastfeeding, your doctor may suggest a vaginal moisturiser or low-dose vaginal estradiol.
When should you see a doctor?
See a gynaecologist if pain continues beyond three to six months after birth, gets worse, or stops you from having sex. Your doctor can examine your scar, look for infection, and refer you to a pelvic floor physiotherapist.

Is it normal for sex to hurt the first time after birth?
Some discomfort is common the first few times. Going slowly and using lubricant helps. Sharp or worsening pain should be checked.
Does breastfeeding cause vaginal dryness?
Yes. Oestrogen levels are low while breastfeeding, which can make the vagina dry and sex uncomfortable until feeding reduces or stops.
Can pelvic floor physiotherapy help?
Yes. A physiotherapist can teach exercises to strengthen or relax the pelvic floor and treat scar tenderness.
