Quick answer

Vaginal estradiol is a low-dose oestrogen used inside the vagina to treat dryness, itching and painful sex after menopause. Because very little reaches the bloodstream, it is much safer than hormone tablets for most women. Improvement usually starts within a few weeks.

What is vaginal estradiol used for?

  • Vaginal dryness, burning and itching after menopause
  • Painful intercourse caused by vaginal atrophy
  • Recurrent urinary infections after menopause (specialist advice)

How does vaginal estradiol work?

After menopause, falling oestrogen makes the vaginal lining thinner and drier. Estradiol applied in the vagina restores the lining, natural moisture and healthy acidity, which makes sex more comfortable and lowers infection risk.

How is vaginal estradiol used?

It is usually used daily for the first 2 weeks, then twice a week to maintain the effect. Cream is inserted with an applicator; tablets are placed with a pre-filled applicator; rings are replaced every 3 months. Your doctor will decide the form and schedule.

Aromatase inhibitors (letrozole, anastrozole)CautionDiscuss with your oncologist; oestrogen may reduce their benefit.
Other vaginal productsCautionUse at different times to avoid diluting the dose.

Who should be careful with vaginal estradiol?

Tell your doctor if you have had breast or womb cancer, unexplained vaginal bleeding, blood clots, stroke or liver disease. Any bleeding after menopause should always be checked, even while using this treatment.

Frequently asked questions

Is vaginal estradiol the same as HRT?

It is a type of hormone therapy, but the dose is very low and acts locally, so it carries far fewer risks than tablets or patches.

How long can I use vaginal estradiol?

Many women use it long term. Your doctor will review it at least once a year.

Can I use a lubricant as well?

Yes. A water-based lubricant during sex can help while the treatment starts to work.
