PCOS and libido: how hormones affect desire
PCOS can affect mood, body image and desire. Treatment and support can help.
Polycystic ovary syndrome (PCOS) can affect sexual desire and satisfaction through hormone changes, low mood, body image concerns, and worries about fertility. Many women with PCOS have normal desire. Treating symptoms, supporting mental health, and open communication with a partner and doctor can improve sexual wellbeing.
What is PCOS?
PCOS is a common hormonal condition that affects about one in ten women in India. It can cause irregular periods, excess facial or body hair, acne, weight gain and difficulty getting pregnant. It is linked to insulin resistance and higher levels of male-type hormones (androgens).
How can PCOS affect sex drive?
- Depression and anxiety, which are more common in women with PCOS
- Body image concerns about weight, hair growth or acne
- Stress around trying to conceive, which can make sex feel like a task
- Side effects of some treatments, such as certain contraceptive pills
What can help?
Treating PCOS symptoms often improves confidence and wellbeing. Regular exercise and a balanced diet help insulin resistance, mood and energy. Counselling can help with low mood and body image. If you are trying to conceive, your doctor may use medicines such as letrozole to help you ovulate, which can reduce the pressure around timed sex.
When should you talk to a doctor?
Talk to a gynaecologist if low desire, low mood or pain during sex is affecting your life, or if you have been trying to conceive for a year (six months if you are over 35). Mention any mood changes too, as treating depression often improves desire.

Does PCOS cause high or low sex drive?
It varies. Some women notice no change, while others have lower desire, often linked to mood or body image rather than hormones alone.
Can the contraceptive pill for PCOS lower libido?
In some women it can. If you notice a change, ask your doctor about other options.
Can women with PCOS get pregnant?
Yes. Most women with PCOS can conceive, sometimes with help from ovulation-inducing medicines.
