Quick answer

Low testosterone can cause low sex drive, erection problems, tiredness, low mood, reduced muscle and loss of body hair. These symptoms have many other causes, so diagnosis needs two early-morning blood tests showing low levels. Treatment depends on the cause and may include weight loss, treating other conditions, or testosterone replacement.

What are the symptoms of low testosterone?

  • Reduced interest in sex and fewer spontaneous erections
  • Erectile dysfunction
  • Tiredness, low mood and poor concentration
  • Loss of muscle strength and increased body fat
  • Reduced body and facial hair, and breast tenderness

What causes low testosterone?

Testosterone falls gradually with age, but a significant drop is usually linked to another factor. Obesity, type 2 diabetes, long-term illness, some medicines (such as opioids and steroids), heavy alcohol use and problems with the testicles or pituitary gland can all lower testosterone.

How is low testosterone diagnosed?

Your doctor will ask about symptoms and examine you, then order a blood test between 7 and 11 AM, when levels are highest. A low result should be repeated. Further tests may include LH, FSH and prolactin to find the cause, plus blood count and PSA before starting treatment.

How is low testosterone treated?

Losing weight, exercising, sleeping well and controlling diabetes can raise testosterone naturally. If levels stay low and you have symptoms, your doctor may prescribe testosterone gel or injections, with regular blood tests. Men who want to father children should not use testosterone, as it lowers sperm production; alternatives such as clomiphene may be used instead.

Don’t take testosterone without tests from gyms or online sellers. It can cause infertility, high blood counts and other serious side effects.

Frequently asked questions

At what age does testosterone start to fall?

Levels drop slowly, by around 1% a year, from about age 30–40. Most older men still have normal levels.

Can testosterone treatment fix erection problems?

It can help if testosterone is truly low, but most erection problems have other causes and respond better to other treatments.

Do testosterone boosters from shops work?

There is little evidence that herbal “boosters” raise testosterone, and some contain undeclared ingredients.
